Assumption of Risks

I HEREBY ASSUME ALL RISKS OF ME AND MY CHILD(ren) PARTICIPATING IN EVENTS AT MyBiblioteka, located at 731 Washington Ave, Brooklyn, NY 11238, including by way of example and not limitation, any risks (foreseen and unforeseen) that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.

I acknowledge that this Accident Waiver and Release of Liability Form will be used by Stysis Enterprises, LLC, DBA MyBiblioteka. I may participate, and it will govern my actions and responsibilities at said activity. I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:(A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from MyBiblioteka, THE FOLLOWING ENTITIES OR PERSONS: Stysis Enterprises, LLC, DBA MyBiblioteka, store owners, employees, volunteers, and representatives;(B) I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned above from any and all liabilities or claims made as a result of attendance and/or participation in events, whether caused by the negligence of release or otherwise. I acknowledge that MyBiblioteka and its owners, employees, volunteers, and representatives, are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting specific and/or all activities on behalf of My

Model Release

I hereby grant permission to Stysis Enterprises, LLC, DBA MyBiblioteka to use photographs taken of me and of my child(ren) at MyBiblioteka, located 731 Washington Ave, Brooklyn, NY 11238 for use in its publications, social media, such as newsletters and brochures, and to use the photographs on display boards, and to use such photographs in electronic versions of the same publications or on the MyBiblioteka.com Web site or other electronic forms or media.

I hereby waive any right to inspect or approve the finished photographs or printed or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the photograph.

I am 18 years of age or older and I am competent to contract in my own name. I have read this release before signing below, and I fully understand the contents, meaning and impact of this release. I understand that I am free to address any specific questions regarding this release by submitting those questions in writing prior to signing, and I agree that my failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release.
